Chart preparation and medical record abstraction are related tasks, but they have different purposes. Chart preparation organizes records so they are available for a scheduled visit. Abstraction captures specified information from records in a structured format for a defined operational purpose, such as quality reporting, payer review, or research.
For example, chart preparation may involve organizing an imaging report with other records ahead of a visit. An abstraction project may capture a documented data point from that report in a designated field, following the practice’s instructions. These tasks should have clear scopes, standards, and handoffs so staff know what is being prepared, what information is being captured, and where questions should go.
For oncology operations leaders, a practical first step is to map the work: identify which records must be available for visits, which data points a separate project needs, and which staff are responsible for reviewing or interpreting clinical information. Agree on documentation standards and escalation steps before assigning either workflow.
CDAS provides medical records support, including retrieving and reconciling records and preparing provider-ready charts. CDAS provides administrative and operational support only; it does not provide clinical advice or make clinical decisions. Clinical questions route to clinical staff.
