Complete, accurate records, before they're needed.
CDAS retrieves records from outside offices, reconciles what arrives against what's required, prepares provider-ready charts, and abstracts clinical data for quality, payer, and research programs, at volume. CDAS-reported: 100,000 medical records abstracted a month.

An incomplete chart delays the conversation that matters.
Oncology histories are long and scattered. When records arrive late or disorganized, the first visit pays for it.
Records from many places
Labs, imaging, pathology, and prior treatment notes arrive from different offices and formats.
Last-minute chasing
Staff spend the day before a visit calling for missing documents.
No view of what's outstanding
Leadership can't easily see which charts are ready and which aren't.
Four records services, one team.
We follow your documentation SOPs, chart standards, and system access rules — agreed during implementation.
- Retrieval: records requests and follow-up with referring offices, hospitals, labs, and imaging centers
- Reconciliation: received records matched against what was requested and what the chart needs, with missing, duplicate, or mismatched documents flagged
- Chart preparation: records organized to your chart standard ahead of new-patient visits
- Abstraction: clinical data captured to your specifications for quality measures, risk adjustment and payer reviews, and research and clinical trials
- Tracking of complete and outstanding items, with exceptions surfaced through your approved process
Illustrative example. Required document categories and turnaround are agreed with your team during implementation.
Clear responsibilities. Accountable support.
Every step follows workflows we define with your team during implementation.
Retrieve
Required records requested from outside offices and followed up on an agreed cadence.
Reconcile
What arrived is checked against what was requested and what the chart needs.
Prepare & abstract
Charts organized to your standard; data abstracted to your specifications.
Report
Leadership sees what is complete, outstanding, and next.
Boundaries that build trust.
What we do is clearly defined — and so is what we don't.
Clear boundaries
CDAS follows your documentation SOPs. Abstractors record what the chart documents; clinical interpretation, coding decisions, and treatment decisions remain with your licensed and certified staff.
Agreed turnaround
We agree on required records, document categories, responsibilities, follow-up procedures, and turnaround expectations during implementation. Exceptions are surfaced through the approved process.
Visibility for leadership
Leadership should be able to see what is complete, what is outstanding, and what follow-up is needed. The reporting approach is defined for your workflow.
What practice leaders ask us.
Do you work in our systems?
Work is performed through the secure, access-controlled systems and permissions your organization approves during implementation.
Do your team members interpret clinical information?
No. CDAS organizes and prepares records according to your documentation SOPs. Clinical interpretation remains with your licensed clinical team.
What is medical record abstraction?
Reviewing a medical record and capturing specific data points, such as dates, results, and documented treatments, into a structured format for quality reporting, payer review, or research. CDAS works from your data dictionary, measure specifications, or case report forms.
How do you abstract data from medical records?
Abstractors work from an agreed specification: which data points to capture, where they can appear in the record, and how to handle edge cases. Each record is reviewed, the documented values are captured into your form or database, and a quality review checks an agreed sample before delivery.
What is clinical data abstraction?
Clinical data abstraction is the same discipline applied to clinical programs: capturing documented clinical data points, such as dates, results, and treatments, for quality measures, risk adjustment and payer reviews, and research and clinical trials. It is what Clinical Data Abstraction Services (CDAS) was named for.
How much volume can CDAS handle?
CDAS reports abstracting 100,000 medical records a month. We scope each project's volume and turnaround with you.
Can you help with a records backlog?
Yes — tell us about the volume and your timeline, and we'll scope a project with agreed turnaround expectations.
One operating model, five services.
Most practices start with one service and expand once the operating rhythm is proven.
Discuss your records work.
Tell us how records reach your team today and where the gaps appear. We'll talk through a workflow that fits.
- Retrieval, reconciliation, chart prep, or abstraction
- Record volume and deadlines
- Scope and turnaround
